Urgent Matters Communities The following ten communities and hospitals were selected to participate in the community safety net assessment and hospital Learning Network. Atlanta, Georgia Grady Health System Grady Memorial Hospital is a 528 bed, public hospital that provides Level I trauma services to residents of Fulton and DeKalb counties. The hospital is a major affiliate of the Emory and Morehouse Schools of Medicine, and is the sole public hospital for the metropolitan Atlanta area. National Center for Primary Care (NCPC) The National Center for Primary Care at the Morehouse School of Medicine promotes excellence in primary care practice and community health programs for all Americans, with a special focus on underserved populations and on the elimination of health disparities.
Boston Medical Center Boston Medical Center is a 547 bed, not-for-profit hospital that provides Level I trauma services. The hospital is the major safety net provider in the city Boston, and is the primary affiliate of the Boston University School of Medicine. Health Care For All Health Care For All is a non-profit consumer health advocacy organization that works with consumers and other organizations to identify the current system's failures and to design solutions to the health care crisis.
Henry Ford Hospital Henry Ford Hospital is a 903-bed, not-for-profit hospital that provides Level I trauma services. The hospital serves as a safety net provider for the city of Detroit, and is a major teaching facility. Voices of Detroit Initiative (VODI) The Voices of Detroit Initiative is a partnership between various healthcare providers and the Detroit Health Department, formed to demonstrate the most cost effective method of providing primary and preventive care for the uninsured and underinsured residents of the city of Detroit.
Elmhurst Hospital Center Elmhurst Hospital Center is a 696 bed, public hospital that provides Level I trauma services. The hospital serves as the medical safety net for the ethnically diverse, urban population in Western Queens, New York. It is also the major teaching affiliate of the Mount Sinai School of Medicine. Northern Queens Health Coalition The Northern Queens Health Coalition is a non-profit organization that works with over 60 member agencies and provides a forum for providers and consumers to collaborate, plan, exchange ideas and build networks to identify gaps and inefficiencies in health service delivery.
Inova Fairfax Hospital Inova Fairfax Hospital is a 753-bed, not-for-profit hospital that provides Level I trauma services to the Washington, DC metropolitan area. The hospital is a member of the Inova Health System, is the sole Level I trauma center in Northern Virginia, and is a major teaching facility. Fairfax County Community Access Program The Fairfax County Community Access Program seeks to develop a culturally competent and integrated delivery system in Fairfax County and the cities of Fairfax and Falls Church.
BryanLGH Medical Center BryanLGH Medical Center is 586-bed, not-for-profit teaching hospital that provides Level II trauma services to residents of Lincoln and Lancaster County, and Southeast Nebraska. The hospital is the main safety net provider for the region. Community Health Endowment of Lincoln The Community Health Endowment of Lincoln was created in 1997 with the mission "to make Lincoln, Nebraska, the healthiest community in the nation." The Endowment places a special emphasis on creating collaborative partnerships to improve the health status of persons at highest risk for the poorest outcomes. The Regional Medical Center at Memphis (The MED) The Regional Medical Center at Memphis is a 355-bed, public hospital that provides Level I trauma services. The hospital is the region's major safety net hospital, serving patients from five different states. It is also a major teaching affiliate of the University of Tennessee College of Medicine. University of Tennessee Health Sciences Center The mission of the University of Tennessee Health Sciences Center is to improve human health through education. The Center's programs are primarily aimed at reducing disparities in the overall health, quality of care, and length of survival among minorities.
St. Joseph's Hospital & Medical Center St. Joseph's Hospital and Medical Center is a 570-bed, not-for-profit hospital that provides Level I trauma services. The hospital serves as a regional referral hospital and safety net provider for the greater Phoenix metropolitan area and Maricopa County, and is a major teaching facility. St. Luke's Health Initiatives The St. Luke's Health Initiatives is a public foundation that focuses on health policy, public education and advocacy activities centered on improving the health of all residents of Arizona.
University Hospital San Antonio University Hospital San Antonio is 642-bed, public hospital that provides Level I trauma services. The hospital is a primary teaching facility for The University of Texas Health Science Center at San Antonio and a member of the University Health System, the sole public hospital system and major safety net provider serving the uninsured of Bexar County. Greater San Antonio Hospital Council The Greater San Antonio Hospital Council is an organization that brings together hospitals, health care providers, and consumers who represent the regions of South Texas. The organization provides leadership in educating, communicating and coordinating health care providers to improve the region's health.
University of California at San Diego The University of California San Diego Medical Center in Hillcrest is a 440-bed, public hospital that provides Level I trauma services to residents of San Diego and Imperial counties. The hospital is the academic medial center for the region and the primary teaching hospital for the University of California, San Diego School of Medicine. Community Health Improvement Partners (CHIP) Community Health Improvement Partners is a collaboration of San Diego health care systems, hospitals, community clinics, insurers, physicians, universities, community benefit organizations and the County of San Diego who are dedicated to a common vision of assessment, collaboration, program development and education. | |||||||
